That’s why we created The Community Driven Initiative — a free delivery program designed to serve seniors, low-income residents, and individuals with limited mobility throughout Medina County.

The Community Driven Initiative?

The Community Driven Initiative is a publicly supported, no-cost delivery service for essential items. Funded by local sponsors, donors, and community partners, our goal is to bridge the gap for individuals who can’t easily leave their homes — whether due to age, health, transportation, or financial hardship.

How The Community Driven Initiative Helps:

  • Prescription Medication
  • Home Medical Supplies
  • Self Care Necessities
  • Essential Supplies

Whether it’s a bag of groceries, a prescription refill, or basic hygiene items, we help make it possible to get what you need — without added stress or cost. Our goal is to restore peace of mind and ensure no one in our community is left behind.

Basic Eligibility

To qualify, applicants must meet at least two of the following criteria:

  • Age 65 or older
  • Living on a fixed or limited income
  • Participating in public assistance programs (SNAP, Medicaid, CHIP, etc.)
  • Living alone or without access to help or reliable transportation
  • Experiencing substantial mobility issues related to health, ability or access

You’ll be asked to provide verification documentation upon conditional acceptance into the program. 

Eligibility is subject to review. Enrollment depends on availability and funding.
